Cancel Residential Service Request

Complete this form to close your account and cancel all services. Regular turnaround time is 5 business days. Please provide your name and address when returning equipment.

Important: Please remember to include your name and address when returning equipment so that the correct account will be updated.

Client Equipment Return Policy:
  • Residents are required to return ALL equipment pertaining to the service provided within 5 business days from the date of disconnect request.
  • All equipment must be returned to our Puichon facility within 5 days of disconnect request. (Returns will NOT be accepted at the Cardinal office or any other Stanford facility)
  • Failure to return equipment in 5 days from disconnect date will be subject to a one-time non-refundable charge of $200 billed to the residents statement.
  • Equipment returned physically damaged or in non-working order due to abuse or neglect will be subject to a one-time non-refundable charge of $200 billed to the residents statement.
