Note: the appointment time selected represents the start time of a two hour window. A technician will arrive to install service within that window. Someone 18 years or older must be there when technician arrives.

• Take your current equipment with you to the new location.
• Please note: A move fee ($140) will apply for transferring service to a new residence or moving equipment to a different TSO (Telecommunications Service Outlet) within the same apartment.
If you are moving out of Stanford housing, please submit a cancellation request.
Existing residents please provide the account number provided by UIT. New residents - please make sure you have access to your new residence before our technician arrives for installation. Our technicians will not be able to gain access to your residence thorough the leasing office.
If yes, please provide a WiFi name for the wireless service. If no WiFi name provided, the default name will be applied.
I acknowledge that accounts 40 days past due will be disconnected immediately. (To restore services, account will need to be paid in full in addition to a $200 restoral fee).
I acknowledge when service is no longer needed a cancel order must be submitted. (A fee of $200 will be charged for each piece of equipment not returned. If a cancel order is not submitted the service remains active. Credit will not be provided for unused service).
