Client Equipment Return Policy:
- Residents are required to return ALL equipment pertaining to the service provided within 5 business days from the date of disconnect request.
- All equipment must be returned to our Puichon facility within 5 days of disconnect request. (Returns will NOT be accepted at the Cardinal office or any other Stanford facility)
- Failure to return equipment in 5 days from disconnect date will be subject to a one-time non-refundable charge of $200 billed to the residents statement.
- Equipment returned physically damaged or in non-working order due to abuse or neglect will be subject to a one-time non-refundable charge of $200 billed to the residents statement.
